They don?t come any better than Dr. Lazaro. Trust me I?ve been to all kinds of doctors and NP?s and this team far surpasses any of them for taking great care of me. He puts in lots of time with me upon visits, he does a full check up each time, makes sure I understand the plan of treatment, follows through with tests and meds immediately.
I had an excellent experience. The nurse who brought me back was incredibly friendly and, upon me asking, she took the time to not only explain, but also demonstrated how BP numbers are determined, and NP Jessica is so kind, warm and engaging, as well as very knowledgeable and easy to talk to. I left in a better mood than I came in with, which says a lot!

She is ALWAYS compassionate, kind, intelligent, focused, and spot-on with assessments and diagnoses. She?s prompt with My Chart messages, reports and responses. She never rushes you or dismisses you until the patient has asked all questions and she has addressed all concerns!!! I am blessed to have Liz as my PCNP!!!!

There aren?t enough good words to describe the care given to me by Dr. Powers. There is nothing more stressful than ??radio silence?? when you are waiting for a response - responsiveness is key to great care and Dr. Powers is an example of excellence. Please clone him.
Mindy Smith is an absolute angel. I have never gotten such great care and have a care provider actually spend so much time with me in person and through My Chart. Even though I am used to an MD for my healthcare, I know that I am getting the absolute best care from Mindy Smith.

My appointment was at 11:30 I got called at 12:30 - not the worst part - Dr Brown just comes in sits at his computer and that's it. I don't ask anything because when I do he acts disturbed and talks so fast and low I can't understand him. I am going to find another doctor because I don't feel that I am getting the care I deserve.
As soon as seven days before an appointment, you can complete eCheck-in through MyChart. You can update information and electronically sign documents before your appointment to make the check-in process faster.
Arrive 15 minutes before your appointment.
Come early to update any necessary information. If you arrive more than 10 minutes after your appointment time, we may ask you to reschedule.
Let us know as soon as possible if you have to cancel.
Please do your best to cancel at least 24 hours before your appointment. If you cancel three times without advance notice, we may dismiss you from our practice.
Payment will be due at the time of your appointment.
Payment is due the day of service, including copayments, coinsurance and any other charges. If your care isn't covered by insurance, you are entitled to a 50% discount.

Prescription Refills
Call and leave a message with your name, date of birth, prescription and strength, along with your doctor’s name and your pharmacy’s name and number. Allow one to two business days for us to process the refill. We won't contact you about the request unless there is a problem.
We cannot refill some prescriptions.
You may be required to see your provider before we can refill your medication. We also do not accept refill requests from local or mail-order pharmacies—you must request these refills.
Lab/Test Results
You can receive timely notifications and view lab results from tests you’ve had done through MyChart. Test results should be available within 7 to 10 business days.
Allow up to 7 days for your referral to go through. Once it does, our office or the specialist's office will call you with your appointment information. If you make your own appointment, please let us know as soon as possible so we can obtain any prior authorizations that may be required.
Forms
Due to the volume of forms we process, please allow up to 10 business days for us to complete them. There may also be a charge for completing your form, or your provider may require that you come in for an office visit for assistance in completing your form.
Immunization Records
Please allow up to two business days on all immunization record requests. If you must call our office for an immunization record, you may be asked to leave a message with the patient’s name, birth date, and contact phone number.
School/Work Excuses
School/work excuses will be issued at the provider’s discretion only when the patient has had an appointment.
Questions for Your Care Team
You can exchange messages with your providers and read doctor’s notes from recent visits in MyChart. Keep messages short and to the point. We will get back to you as soon as possible.
After hours there will be an on-call provider who will be available for urgent needs that cannot wait until the next business day. If an emergency occurs after hours, please have paper, pencil and pharmacy phone number available. Prescription refills will not be handled by the on-call provider nor will pain medications, controlled substances or antibiotics be prescribed to your pharmacy.
