Financial Support for Qualifying Patients
Patients with limited or no insurance coverage can apply for financial assistance with covering the cost of their care.

Complete the application and provide the required documents for income verification.

Required Documents
- Photo ID and Social Security Numbers — For all eligible household members.
- Income Verification — 2 months’ worth of paystubs.
- Complete Bank Statements (All Pages) — For all applicable checking and saving accounts.
- Rent Receipt or Current Lease
- Proof of Food and/or Housing Assistance (e.g., SNAP, WIC, Section 8 Housing)
Eligibility
We use the Department of Health and Human Services’ standards to determine a person’s eligibility for financial assistance.
Likely Eligible
Patients who qualify and are receiving benefits from the following programs or who are in the following categories may be presumed eligible for financial assistance:
- State Medicaid Programs — Must be verified and entered into the system for record keeping and claim submission.
- Food Stamps — The U.S. Department of Agriculture Food and Nutrition Service Food Stamp Program (SNAP)
- County and State Relief Programs — Financial assistance with housing, utilities, food and WIC nutrition assistance.
- Homeless Patients
- Deceased Patients — Unpaid balances of patients who are deceased with no confirmed estate after one year.
Not Eligible?
If you don’t qualify for financial assistance, you can still set up a payment arrangement to pay your bill over time.
